Bichectomy

Bichectomy means the removal of fatty tissues in the cheeks. Bichat fatty tissue in the cheeks may cause a larger and rounded and chubby face in some patients. Although there is no excess fat in other parts of the patients’ body, this look may cause them to be unhappy. With the removal of volume of 2-3 cc fat from each cheeks by bichectomy, cheekbones become more distinct and face looks thinner and more elegant. Bichectomy is a 30-minute procedure under local anesthesia. The 1 cm incision is made on the cheek of the upper molar tooth and the fatty tissues on the cheek are reached and removed. The incision sites are repaired with self-absorbing sutures. We advise our patients not to eat something for up to 3 hours after the surgery, then they can continue their normal diet. The swelling of the cheek area after bichectomy disappears approximately 2 weeks later after the surgery and at the end of the first month, our patients can see the results of the surgery clearly.

What is Bichectomy?

Bichectomy or cheek fat removal is the procedure of removing fat by making a small incision on the inside of the cheeks, near the fat bags. This procedure is usually performed under local anesthesia. The total procedure takes approximately 30 minutes. During the surgery, an incision of approximately 1 – 1,5 cms is made in the mouth. Bichat pouches with a volume of 2,5 – 3 cc are removed with this incision. Then, the wound is sutured with 1 or 2 self-melting stitches.

 

Who is a good candidate for a Bichectomy or Hollywood Cheek Surgery?

Bichectomy surgery is performed on average weight people who would like to improve their look by a more defined face and more apparent cheekbones. Patient selection criteria is highly important for this procedure. It is recommended for patients who are over 18. If cheek thinning procedure is applied to any patient who already has thin cheeks, this would cause negative results in their old ages. For this reason, an ideal candidate has a round face with bigger cheeks which is not proportional to her/his face.

 

 

What are the results of cheek thinning surgery?

After the surgery, the cheeks get remarkably thinner. The patient will have a more elongated appearance with aesthetics in the facial features. Because the fatty tissues are removed, the cheekbones will appear big and contoured.

 

Can I see the final results right after the surgery?

After cheek thinning, the incisions in the mouth are sutured with self-melting stitches. For this reason, wound dressing or stitch removal is not required later on. We recommend the patient not to eat for around 3 hours after the surgery. We also provide you mouthwash (oral spray) to accelerate the recovery process after the surgery.

 

Is Bichectomy and Hollywood Cheek the same procedure?

Bichectomy is the name of cheek fat bags removal procedure. Hollywood cheek is the name to represent the new and aesthetic appearance of cheeks after fat bag removal.

 

Can bichectomy be combined with other surgeries?

As we mentioned above, more sharp lines on the cheeks, contoured jaw lines and thin neck have been popular trends. When the cheeks and face are thinned, cheekbones will become bigger. In order to have more contoured cheekbones, bichectomy can be combined with cheek fillers procedure or cheek augmentation with fat transfer. Other than that, after cheek thinning surgery, jawline procedure can help have a more contoured and aesthetic jaw.

 

Which surgeries are frequently combined with Bichectomy?

Rhinoplasty and vaser liposuction for jowl can be combined with bichectomy. When cheek thinning is combined with jowl vaser liposuction, both the cheeks and neck areas get a thinner look.

 

Are there any dangers or side effects of bichectomy surgery?

If the surgery is performed on the ideal candidates, there is no danger or side effect. However, if it is performed on patients who have thin cheeks and long faces, the cheeks will be sagging. Thus, it will give an older look to the patient.

 

How does the doctor decide on the amount of buccal fat to be removed?

The fatty tissues on the cheeks have different appearance and structure than the other fat tissue types. When the incisions are made in the mouth, the fatty tissues will be apparent. The best and most wanted result is acquired by removing the buccal fat which can be easily pulled away without any pressure.

 

What is the technique of removing buccal fat pads?

An incision of 10-20 mm is first made inside the mouth, just above the upper molars and in the inner mucosa of the cheek. The scar will thus be totally invisible since it is hidden in the mouth and the suture will be made by means of absorbable threads (which will fall between 7 and 15 days).

 

After this incision, the buccal fat pads is identified and only the excess that causes the rebound is removed.

 

You should know that this area is rather complex because it contains many very fragile veins and multiple branches of the facial nerve that should not be damaged. 2-3 cc fat tissue is removed from each cheeks by bichectomy.

 

How is the procedure process?

Before the procedure :

It is imperative to stop smoking 1 month before the procedure, and in the 10 days prior to the procedure, no anti-inflammatory drugs or aspirin should be taken. The dental condition must also be correct, without an outbreak of infection.

 

During the procedure :

Anesthesia is local most often. And the hospitalization takes place on an outpatient basis (entry in the morning and exit in the evening) with a return home on the same day. The intervention lasts 30 minutes.

 

After the procedure:

No dressing is necessary after the operation, but the application of ice on the cheeks is strongly recommended in order to reduce the swelling which may be important during the 7 days following the operation.

 

Patients are free to eat any food after 3-4 hours, and they apply mouthwashes for 7 days. It is also important to know that the pain is not very strong and is relieved by simple analgesics ; bruises are rather rare. The scar inside of the mouth closes 2 days later.

 

The professional interruption is from 2 to 5 days (this is not a work stoppage since it is a cosmetic surgery operation).

 

The result of this procedure is definitely visible after 3 months after the operation, when the edema is completely absorbed.

 

In conclusion, if a diet or a balanced diet fails to make you lose cheeks, then the only trick you have left is surgical with an operation of the buccal fat.
