Rhinoplasty

The nose is located in the middle of the face and most distinctive organ when looking at face. In rhinoplasty, problems that prevent breathing such as deformities in the nose and deviation are tried to avoided. Simultaneous procedures for two different targets can be performed in rhinoplasties.

 

One of these goals is to restore the deviation on the nose to provide breathing and allow to physiological functions, removal of bone, cartilage or concha obstruction which prevent breathing. The second goal is to create a nose shape that is suitable for the face structure of the patient.

 

I perform my rhinoplasties with taking these two goals into consideration with the same seriousness. In order to achieve good results in rhinoplasty, good planning is required. For this purpose, I try to explain the changes that I want to make to my patients before the surgery and show the result that I aim at the end of the surgery by using every developments of technology.

 

 

Although I prefer open surgery technique in rhinoplasty, it is frequently used in closed rhinoplasty method in appropriate patients. The most beautiful and most positive development for me in recent years is the ultrasonic device (piezotome) became an integral part of the surgical technique. I perform bone forming procedures now with ultrasonic rhinoplasty technique which we previously perform with the help of the hammer, chisel and rasp. The most important feature of ultrasonic rhinoplasty is that the device used only affects the bone tissue, does not damage the cartilage, mucosa and vascular structures of the nose, it both reduces the bruising and edema significantly and provides a shorter and faster recovery for the patient.

 

I prefer to perform my rhinoplasty operations under general anesthesia. Hospitalization of our patients for one night is very important in terms of providing good care after surgery as well as patient safety. I prefer to remove the splints and tampons from the patient in 7th day but this situation differs from patient to patient.

 

With the consent and informative forms we sent to our patients before the operation, we aim to our patients have all kinds of information about rhinoplasty that takes approximately 2-2,5 hours and postoperative period.

 

 

Is there an age limit for rhinoplasty?

It is recommended that teens desiring cosmetic and functional nasal surgery wait until they have gone through puberty. For female patients, this means waiting until they’re 17 years old and for males, waiting until they’re 18. Patient under 18 can have a surgery only with parental permission while adult patients can get a surgery procedure by informed consent. In ClinicPlast, we prefer to perform surgery for patients above 18.

 

Why is skeletal maturity important for rhinoplasty surgeries?

Three types of tissues are shaped during rhinoplasty; nasal bones, nasal cartilage, and nasal skin. Nasal cartilage and skin can reach maturity at early ages while nasal bones continue growing until 18. For this reason, it is preferred to make a change on nasal bone structure only when the nose has reached its mature size and length. Otherwise, any change in nasal bone structure will not be permanent.

 

Can I have a rhinoplasty surgery if I have a crooked nose?

Patients requiring rhinoplasty might have aesthetic concerns as well as nasal septum deviation. During the surgery, the defects in nasal bones and cartilage as well as curvatures are fixed. The main purpose of rhinoplasty lies in improving the shape and structure of the nose to have a contoured facial appearance while fixing any respiratory problems.

 

 

Why can I only breathe through one nostril?

We have several patients who complain about a blocked nostril. Commonly, people only breathe out of one nostril at a time. However, it might change which nostril is active during breathing. In case one nostril is always blocked, this may be caused by an anatomical problem. For any patients who always have one nostril blocked, we recommend to have a tomography scan prior to the rhinoplasty surgery.

 

Should I stay at the hospital after the surgery?

Rhinoplasty surgeries are performed under general anesthesia. It is recommended to stay at the hospital in order to release side effects of anesthesia, and to feel better.

 

How bad is pain after rhinoplasty?

The main reason for severe pain after the surgery was a consequence of a conventional method that cuts the nasal bone with a hammer and chisel during the surgery. Today, piezo rhinoplasty is the most advanced technique available for nasal bone reshaping as it is performed with an ultrasonic device. Rhinoplasty using Piezotome is a pain-free procedure.

 

What are other advantages of ultrasonic rhinoplasty?

Ultrasonic rhinoplasty is a pain-free procedure as nasal bone is reshaped by an ultrasonic device. Because the device only affects nasal bone tissues, any veins, nerves or other soft tissues are not damaged. As a result, the patient will have less swelling and pain which will also help fast recovery after surgery.

 

Why is 6 weeks post-op important?

Usually, it takes 6 weeks for the bones to recover. 6 weeks post-op is highly important for bone recovery.

 

Is it mandatory to use a thermoplastic splint and a silicone tamponade after a rhinoplasty surgery?

After a rhinoplasty surgery, a thermoplastic splint is applied if nasal bone structure is reshaped. The purpose of using a splint is to support proper recovery of the nasal bones. Also, if a crooked nose is fixed, a silicone tamponade is placed. The tamponade helps both protect the nose and prevent bleeding. Only when a nose tip-plasty is performed, there might be no need neither for a splint nor for a tamponade.

 

Does removing nose splints hurt?

This is one of the most frequently asked questions by patients. Silicone tamponade and thermoplastic splint removal is a pain-free procedure.

 

Is it more difficult to have a nose job if I had rhinoplasty before?

Yes, it is more difficult for a patient to have a nose job for the second time. This is because the nose might have left sticking during the recovery process of the first surgery. Especially in the first surgery, the cartilage may be highly damaged. In such cases, ear cartilage or rib cartilage is picked in order to reshape the nasal cartilage.

 

How long after a broken nose can I get a rhinoplasty surgery?

In case of a broken nose, a rhinoplasty surgery is performed one year after it is broken. In one year, it is expected that swelling goes down and the recovery process is completed.

 

Is it necessary to harvest ear or rib cartilage during a nose job for a broken nose?

It depends on the trauma intensity. If the nose is highly damaged and the nose cartilage cannot be used, it is required to harvest cartilage from ears or ribs. However, in most broken nose cases, it is not necessarily required to perform rhinoplasty by picking cartilage.

 

Does a 2D or 3D simulation reflect the surgery result?

Simulations are significant in terms of measuring and estimating whether the patient’s and the doctor’s preferences match. However, the simulation would not indicate the actual surgery result 100% correctly. This is because intranasal changes can be monitored during the surgery whereas it cannot be fully under control in the recovery process.

 

How long does swelling last after the surgery?

80% of the swelling goes down in 2 months after rhinoplasty surgery. It might take up to 1 year for the remaining 20% of the swelling to disappear.

 

How long does it take for my nose to reach its final shape after rhinoplasty?

After rhinoplasty, it takes up to 1 year for a nose to reach its final shape. After 1 year, the nose is permanently in its final shape.

 

How soon can I have revision rhinoplasty?

Before you consider revision rhinoplasty, your nose should be healed fully from a previous surgery. Full recovery takes around 1 year. Thus, you may have a revision rhinoplasty after 1 year from your first surgery.

 

Should I prefer open or closed rhinoplasty?

This preference depends on the severity of the defect, septum deviation, and any trauma caused by a former nose job. You may prefer a closed rhinoplasty only when nose tip or nose hump will be reshaped. However, in cases of severe defects or second-time nose jobs, open rhinoplasty is far more advantageous. Also, the doctor’s preference is highly important while deciding on the technique.
